Payment Options

Payment OptionsWhen do patients pay?

  • Patients with insurance that we carry usually have a co-pay they pay on the day of their visit. Then they have 30 days from the date their insurance pays its part of the charges to pay the balance that they’re responsible for.
  • Patients without insurance or private pay patients who have insurance with companies we don’t contract with have 30 days to pay for their charges after the charges are entered.
  • Cosmetic patients pay for their procedures the day they get them.

What forms of payment are accepted?

  • Checks
  • Credit cards (Visa, Mastercard or American Express)
  • PayPal for online bill pay which accepts credit cards (Visa, Mastercard or American Express), eChecks and bank transfers.

Cosmetic Payment Plans

You can sign up for CareCredit Healthcare Financing to finance your cosmetic procedures for amounts between $100-$3000. Our terms with CareCredit are a no interest payment plan if paid in full by the required due date at 6 months.

Medical Payment Plans

Don’t let your high deductible or lack of insurance stop you from getting the care you need. We have payment plans available for medical patients who owe between $100-$3000. You can set up your payments to be automatically withdrawn from a checking or savings account or from a credit or debit card. The payments are typically set up on a monthly basis but can be set up on a weekly basis if you prefer. The number of payments depends on the amount you owe.

How to sign up for a medical payment plan

To sign up for a payment plan just fill out our Auto Recurring Billing Authorization Form (PDF) and either drop it off, mail it in or fax it back to us along with any required paperwork that’s asked for on the form, such as a voided check, savings deposit slip or credit card photocopies. Your first payment plus the set-up fee will be due at the time you set up the plan.

Please note that a credit check will be run before your payment plan is set-up if you are applying for $500 or more in credit. If your credit is denied and we can’t set up a payment plan for you, all but $10.95 ofyour set-up fee will be credited towards your remaining balance.

Unpaid or overdue bills

What happens if you can’t pay your bill on time? Unfortunately, there are some accounts we have to send to collections after making every possible effort to work with the patient. If you’ve received a collections notice, you can contact Evergreen Financial Services online or at 509-943-2224 to discuss your account further.
