For your appointment, please:
- Fill out and bring in the forms we’ve mailed you. Or if you didn’t get them in the mail, you can get them from our forms page.
- Arrive at least 20 minutes early, especially if you aren’t bringing in your paperwork already filled out. Otherwise, the time spent on registration and paperwork could cut into your visit with the medical provider.
- Bring a list of any prescription and over-the-counter medications you are currently taking.
- Bring a list of any prescription and over-the-counter medications you are allergic to, and the reaction you have had to
them.
- Bring the results of any relevant medical tests, pathology reports or medical records you might have or a list of the doctor(s) we should get them from.
- Bring your insurance card (s) and driver’s license for us to scan.
- Bring a book, magazine, iPod, etc. just in case we aren’t running on time. (We do, however, tend to run on time most of the time, so please don’t come late thinking we’ll be running late or you may need to reschedule your appointment.)
- Bring snacks or money to use at our coffee cart if you are going to be here for an extended appointment, such as Mohs surgery.
- Bring a sweater: Our office can get chilly even in summer.
- Bring our patient porter’s phone number (551-2320) with you if you’ll be needing assistance from your car into the building.
- Print out a map/directions from our web page to bring with you if you’re not sure how to get here.
- Do your homework. On our website, we have a list of frequently asked questions, sections on some of the skin conditions we treat, our medical treatments, cosmetic services and products available.
